Quads (Vastus Medialis)

The vastus medialis is a powerful knee extensor, so it can thrive under substantial loading. But because it works as part of the quadriceps rather than in isolation, the best range is one that lets the quads approach their limit before breathing, bracing, balance or general exhaustion ends the set. 8–15 reps provides enough load for high mechanical tension while still giving the vastus medialis enough repeated knee-extension work to reach deep local fatigue.

Going much lower makes quad training increasingly dependent on very heavy loads, where joints, spinal loading and whole-body fatigue become more expensive. Going much higher can work extremely well on stable machines, but on demanding compound movements the set can become systemically exhausting before the quads are truly finished. The middle ground works exceptionally well across the movements most useful for building the vastus medialis.

Quad training carries a high fatigue cost. The exercises that load the vastus medialis hardest also tend to involve large loads, substantial knee forces and considerable whole-body effort. Stopping at 1 RIR captures nearly all of the productive end of each set without repeatedly forcing technique and performance to deteriorate at true failure. Five hard sets is therefore a better trade than trying to extract maximum stimulus from fewer all-out sets.

At 1 RIR, each productive set contributes roughly 4 GPRs. Five sets × ~4 GPRs = ~20 GPRs, reaching the session target without unnecessary failure work.

Crucially, the vastus medialis does not have its own separate GPR budget. It shares a single quad budget with the vastus lateralis and rectus femoris. You cannot meaningfully train knee extension for one of these regions while leaving the others completely untouched. A vastus-medialis priority therefore means biasing more of the shared ~20-GPR quad stimulus toward movements and mechanics that favor it, not adding another 20 GPRs specifically for the medial quad.

Every 3 days

Hard quad training combines substantial local muscle fatigue with heavy joint loading and some of the highest systemic demands of any hypertrophy work. About 72 hours gives the quadriceps, knees and supporting musculature enough time to restore high-quality force production before another full stimulus, while still allowing the quads to be trained frequently enough for rapid progression.

The vastus medialis also shares its recovery clock with the rest of the quadriceps. A squat or leg extension biased toward another quad region still substantially loads the vastus medialis because all three major superficial quad regions contribute to knee extension.

Think quads every three days, then choose the regional bias of the exposure. Changing from a vastus-medialis-biased exercise to a rectus-femoris or vastus-lateralis-biased exercise does not give you a fresh recovery clock.
